We are excited you have chosen to nominate some very deserving people as a recipient family this upcoming holiday season!

To help with the nomination process, here are some FAQ's about our program and what happens after you nominate a family:

We will keep all submitted information confidential. For our records we will need to collect your information, but rest assured we will never share or sell anyone's information under any circumstance.
Please complete this form to the best of your ability. We sometimes have problems contacting nominated families, so the more information you provide us, the better.
We will not tell a nominated family who submitted the nomination under any circumstances.
We cannot guarantee that the family you have nominated will be chosen. Unfortunately we receive many more nominations than we are able to provide for, but every year our program grows and we continue to help more and more families!
If the family you have nominated is chosen we will make three attempts to contact them with 24 hours between each attempt. We will use every method of communication you provide to us. However, if the nominated family fails to respond to our attempts at communication, they will be disqualified.
If at any time we discover fraudulent information was provided to us, both the person who submitted the nomination and the family nominated will be disqualified.
Nominations will be accepted beginning October 1, and will end November 10. We will not make exceptions to this rule.
