We are so excited to share the magic of the holiday season with you and your family!
We want you to be as informed about Christmas Angels, and our policies, as possible so we have put our most commonly asked rules, policies, and FAQ's here for you to print/save/screenshot for reference.
Please read the rules/policies/FAQ carefully before you complete and submit this form! Failure to follow the rules and guidelines below could result in your family not receiving the appropriate items requested/needed by your family.

Fields in the form below that are required have an asterisk (*) beside them, but we encourage you to provide us as much information as possible.

Your information is always kept confidential. We will never share or sell your family's information with anyone!

Your Christmas Angels admin has assigned you a Family Number. It is important that you put that number at the top of this form - it is how we will identify your family on shopping day to preserve your anonymity. We want to make sure the correct items are delivered to your family!

Christmas Angels defines a 'family' as parents/guardians, and the minor child/ren under their direct care/responsibility. This could be parents and their children, grandparents with grandchildren, etc. We do not discriminate based on the type of relationship you have with the minor child/ren you are responsible for. As long as the child/ren is/are under your direct care/responsibility they are considered a member of your family.

Please provide accurate and current contact information. If your family is chosen we will reach out to you using all methods of communication you provide. If you fail to respond after three contact attempts, your family will be disqualified. It is very important you provide accurate and current contact information.

YOU ARE RESPONSIBLE FOR PICKING UP YOUR GIFTS ON "DELIVERY DAY". We will provide you the time and location as well as a few reminders leading up to the day. You may designate someone to pick up your items on delivery day if you can't make it. If this is the case you must tell us who your designated pick up person is. Gifts will not be released to anyone not designated. Failure to pick up your items could result in your family to receiving your gifts.

PHOTO ID WILL BE REQUIRED TO PICK UP ITEMS. This will apply to EVERYONE (including you).

This form asks you to provide us with three "wants" and three "needs" for each child under 18 in your household. Please remember this does not mean each child is going to receive everything requested. We do our best to fulfill as many items as we can, but we are a volunteer/charity organization and items are purchased and donated through various means (cash donations, item donations, fundraisers, etc). The items that are purchased for your family are done so as our budget allows.

Please be detailed when asking for clothing items! We need to know what size your child wears, what color(s) they prefer, or what to avoid (aka elastic round the wrists/ankles, etc).

Please tell us if your child cannot have certain toys. If your child has sensory issues (sensitivity to sounds, noises, etc) please note this in the wants or needs section for that child so we can keep this in mind when purchasing items for that child.

Please be mindful of the fact that the items you're asking for are purchased with donated funds. Asking for high priced items such as gaming consoles, iPads, laptops, major household appliances, expensive home repairs, etc are beyond our budget. We do not ask donors to purchase expensive gifts for our recipient families. If you ask for a high-priced item (ex: a PS5, a laptop computer, etc) we will disregard this request and it's possible your family may not recieve what is truly needed.

We will not fulfill gift requests of cash.

Items, goods, or services received are accepted as-is. We do not have the ability to "upgrade" or exchange items, goods, or services for something larger or of more value - nor will we ever ask a donor to do so. Christmas Angels is always thankful to all of our donors for their generosity, and we happily accept whatever our donors are willing to provide for our recipient families.

Items, goods, or services cannot be returned or exchanged. Christmas Angels is a by-donation organization. We do not maintain a storage unit of gifts. This is why we ask that you be very detailed when providing us the information requested on the form. We want to make sure that the items donated to your family are exactly right.

Fraudulent information submitted to Christmas Angels will result in the termination of the relationship between the recipient family and Christmas Angels.

Threatening, hostile, or disrespectful words or actions toward any Christmas Angels volunteer will never be tolerated and will result in an termination of the relationship between the recipient family and Christmas Angels.
